Physical Examination
Students registering for Athletics must have a current WIAA physical card or alternate WIAA physical card submitted to the Administrative Athletic Assistant, Mrs. Stowell, at least 5 days prior to tryouts or the start of their season. Physicals must be dated April 1, 2022, or later to be valid for the 2023-24 school year. Download the physical form by clicking the Activities Team Registration icon and complete a family account and follow download instructions.
